I can't find a .pdf manual for MOTU MachFive, and Google is failing me on this question.
MachFive 3 can read several instrument formats, including .ufs (UVI Workstation and Falcon). What is its native format or formats -- the one(s) that it saves new instruments to? Specifically, does it write .ufs files that can be read by Falcon and/or UVI Workstation? Can I use MachFive 3 as an intermediate step to get EXS, Gigasampler and unincrypted Kontakt instruments into Falcon? What about earlier versions of MachFive?

It only saves its own format - it's not a format converter. UFS files can only be created by UVI in their library encoding procedure (however Falcon does read all MF3 files).
