change midi patch names?

Is there a way to edit or change the list of midi patches from GM Midi to a user list? I cant find any way to select other than the GM list.

A simple text "ini" file or something that I could change bank name would be enough...


PS - sorry for the newbee question

Also - isnt there any plug-ins / VST for midi effects???
