Many wave editors will let you modify samples arbitrarily with a pen type cursor. Big ol discontinuities in the signal (adding blank spots where the signal goes from full to nothing over a single sample) and less than ideal audio quality will get you in the ballpark. Pasting digital silence in small amounts in semi random spots is also fun. There are some good tools out there that rebuffer your audio and then "break" the buffer (think overloaded asio stream or a stuck buffer like a very bad daw crash). There's also stuff like the telecom settings in speakerphone, or the various different dac and encoder settings in plogue chip crusher or the different ways of reading out the buffer data in SC permut8.
