Sustainiac makes a model that clips to the headstock. It requires an extra "lead" (ie. cord) going from your headstock to the driver, but apparently your whole guitar springs to life.
A fellow who goes by PSW on ProjectGuitar.com is working on a Sustainer, and has already created working (and good-sounding!) prototypes. For the interested, just go to projectguitar.ibforums.com and do a forum search for "sustainer" with "psw" as the author.

Wow, this thread grew. I'll try feedbug.
I'm not a guitar player and play all of my guitar parts on a keyboard with a couple of different multisamples then through an amp sim. What I'm going to do is play around with my effects chain and try to get it to sound as much like real guitar feedback as I can.
