Tip: have duplicate forms open - the first one could be just for marking down all tracks you might want to assign a vote your 1st listen through, the second one could be for draft votes for your 2nd listen through of the tracks you marked down, and the 3rd would end up being your submitted form. I just used 2 duplicates, but in retrospect 3 would have made it easier.
@bjporter - Even if I wanted to fill it up more...I'm afraid that is the extent of my sound design skills. Took me forever to get those sounds out *cry*, and I am so awed when I hear other tracks that were submitted.

RULES:

The form is created in a way to allow multiple 5s 4s 3s etc. I missed that rule above and ended up voting several 5s some 4s some 3s not knowing what to do
Had you just had a drop down with 5 boxes, that would have made it easier... and obvious.

5 is not enough tracks I would like to vote for
how to choose the 5 ? there is so many good stuff or interesting... I will need more than one listening to choose. And it is really long to hear 60+ tracks
I miss time to work with feldspar (or to buy vst)

Hmm... aside from a drop down list, maybe it would be easier to scale the amount of points that are allowed to be given with the amount of entries that are in a particular month's contest?
For instance, a month with over 40 entries might see 6 points as best instead of 5? Not to shame the current voting system, just thought I would share an idea.

You are using Google forms, right? For me, it works without any trouble: Add an item, and for question type, select "Choose from a list". @VitaminD: No problem, just vote again, I've removed the one with errors
@Davias and Sovarozum: Believe me it will not be easier if you have to choose more than 5, we had more before and most found that even more difficult. The scalable idea with the slightly increased number for huge fields like now is not bad though, it's like a bonus point to a favorite 'outsider'. I will include this as an option in the next rules vote and see what people think then.

Just thinking out loud here, but when people feel that picking only 5 tracks is to hard (I can imagine), why don't we change the way we vote completely?
I would love to be able to give 1 to 5 points (stars) to every track I listen to.. listen to it.. and decide for yourself what kind of rating you want to give that track. I could end up with 3 tracks I believe are amazing and all deserve 5 points.. so I give them all 5 points.
Maybe there are 6 tracks that I think are good, but nothing special, I could give them 3 out of 5 points.
So, give ALL tracks a rating between 1 and 5 points/stars and in the end, it's very easy to see who received the most points. (and the second most, third most, etc..)
Many time in previous OSC's I wanted to give points to more than 5 tracks, but I couldn't. That problem would be gone in this scenario.
In my opinion, this would be the most fair way of voting.

For example, if there was only 8 tracks in the contest.
Track 1 **
Track 2 ****
Track 3 *
Track 4 *****
Track 5 *
Track 6 ***
Track 7 **
Track 8 *****
So we could vote for all tracks, each star means +1 point.
This way all tracks could be voted and in the last moment, it is easily to see the winners, the ones with more stars.
Well, this would be nice when scoring. This way you don't have to break your head about the best ones. You just vote, and the sheet scoring system would add all these points.
