Frequency/pitch chart needed up to 12khz

Hey guys :)
Im looking for a frequency/ pitch chart,
( you know A4 pitch =440hz, ect)
I need it to go beyond C8/4186hz (top note of piano)

This will give me a guide for eq'ing 2nd & 3rd order harmonics in a mix.

Thank you scot for your chart, unfortunately it only
goes up to 1.047khz (C6).
I would like something up to 12khz if at all possible.

what about some multiplication (x2)?

freq = base_note_freq * 2^(relative_note / 12)
for instance let base_note_freq be 440hz, if relative note is 0 you get the frequency for an A, if relative note is 11 you get the frequency for the A# almost one octave above 440hz, etc.
