i have made one with SE, but not released yet. PM me and i'll send you the dll per mail.
Make sure your Host can send MIDI Notes to your FX ( Energy XT does this, dunno about others )

Or you could simply use Niall Moody's Midi ADSR. Which funnily enough is a midi controlled ADSR...

An envelope follower with a sustain stage really wouldn't make a whole lot of sense, as there really isn't any gate signal to hold the envelope. The envelope gets triggered when a signal passes a threshold, but the signal in most cases drops below that threshold right away.
The only thing that would work is MIDI triggerable envelope built in to the filter, where the gate was held until a MIDI note off message. Maybe that's what you want, but it would be useless as an autowah filter.
The issue with the NDC ADSR, which can be very useful, is that it wouldn't modulate the cutoff on a filter plugin (I don't think it spits out MIDI CC values) like a regular synth filter would.
