I use CutePDF Writer but it requires you to install GhostScript so I don't know if it meets the minimal install/bulshit requirement

PrimoPDF is no B.S. Just install and it becomes a printer (like CutePDF) device.
None of the free versions give you the output quality of a true Adobe-created PDF, and I haven't found one that allows editing of bookmarks, etc., (haven't checked the above link yet), but they do the trick.
If you have OpenOffice installed, it gives an 'export to pdf' option right in the file menu, which is what I use for creating the BetabugsAudio manuals.

Another shout for this one. It displays a web page each time you print to the driver but that is easily closed. It has been a while since I used it but I have had good results.

any advice on handling images, related to cutepdf or in general? there's some size reduction, eg. the pdf displayed concept of images at 100% is ~70%
is there some non-arbitrary size i can enlarge images to in order for them to display so people can see the crap w/o it looking humourous?

BHcomp + HTMLdoc, see http://www.asseca.com/bhcomp/w-0.htm.
In HTMLdoc an image-width 520 - 600 pixels works best.
