What format is a ringtone in?

I know - I hate them too but am planning to make one just for fun and subversion. Are they sample based or some sort of midi? (I'm thinking of that Frog shit for example)

the f**king frog shit is a "TrueTone", ie: audio based not MIDI. Current state of the industry means these could be delivered as ADPCM WAVs, MP3s, AMRNB, AMRWB, RMF, XMF, SMAF and probably more that I have forgotten.
General MIDI should work on all Nokia phones, but polyphony is very restricted on the cheaper models (typically 4 voices) so don't expect too much! You can also make MIDI based SMAF ringtones which can sound a lot better, but if I told you how to do that I would have to kill you..

not exactly to the point (it surely is *.crp - format
) but the little Anvil Studio I got offers easy making old handy-crappy-making. You play a midi-file and convert it. Costs some 19$ additional to Anvil Studio, and at least on the website you should find info of how you do the thing (www.anvilstudio.com). The guy who runs it is most friendly by the way and surely will answer any question.

I guess it depends on the specific phone you use. I believe most are capable with midi files unless the manufacturer only uses some sort of weird-ass proprietary format. My LG VX6000 accepts midi files and mp3's with their extension changed to .mid so instead of rinky-dink GM tones on all of my rings, I've got some actual music too.
