I just recieved a good question via email, so I thought I'd share it with you all in case you were wondering the same.
For the duplium form, there is a field titled "Company". I chose to fill this in with "Chrysalid:Musicians for Katrina Relief". For the field titled "Job Title", I simply put "artist". It should not matter too much if you put something else and have already mailed the forms away, but for those that have yet to fill theirs in, let's use this format for consistency purposes.

But seriously, I found out on Sunday about the Duplium form. I guess they usually don't sweat it because they are used to getting either a single artist's cd or a compilation from a label, where there is only one signed form used by the agent/agency. I, in my inexperience with such things, did not see it coming. xRAVENx and I thouroughly discussed the matter through the weekend and decided it best to have our own form as well so everyone in the project was cleared. I originally planned to sign the Duplium document as the agent, but after thourough discussion, decided that it was not a wise idea.
I do apologize about the suddenness of it, but what can you do? All we can do now is move forward and try to get those forms in as soon as possible.

Yeah, I think scanned contracts are fairly common, at least until the hardcopies arrive.
