One way you can do this is with the Midi Out plugin.
Assign it to a track, make sure it's output port
is the same as the input port on the VSTi you
want to change programs in.
Hit record and change patches where you want them to occur.
You'll have to ignore the patch names in the Midi Out
plugin because they're hard wired to GM patch names
and will be irrelevant to the patch names in your
VSTi, unless it's a GM synth.
You can also right click on the Patch Number
select "Edit Events" and draw the patch change in.
You can tell which patch number the cursor is at by
watching the hint bar.
It's a bit wierd....but it works.....

Yes , and even autmate them via "Create Automation Clip"
