If it's just the chords, it would be a "chord chart". A "leadsheet" would also have the melody and maybe the lyrics.

Correct!

A peice thrown in front of an improv musician (typically jazz) would be reffered to as a "chart".

Actually I'd love to have my chops and be able to follow 'charts' ... someday!!!( "charts" that's what the local jazz dudes here call them )
