Lead Sheet is what I would call it - however, in the past this did mean that it included a musical notation version of the melody - but I believe many people (myself included) refer to the lyrics of the song with chords put over top of the words as lead sheets these days.

'Chord chart with lyrics' is another perhaps more proper way to define it.

One thing that it definitely isn't is tablature. Tablature is an alternative notation methodology primarily designed for and used by guitarists.

It's also a "fake sheet." Or it's "a page that shows the chords." Or it's a "here, play this." It's really not going to be a topic of much discussion, in a situation where such things are used :-) After the first time through the tune, it's a beer coaster.

Seems that it depends on what your musical background is (classical, jazz, rock, etc.), but at the end of the day it's:
"a page that shows the chords."
