You write that the mouse freezes when you open tablet2midi - is that mouse using the tablet or is it a USB / ps2 mouse independent from the tablet? If the mouse is using the tablet then the behavior is normal, because tablet2midi is taking control of the wintab driver - as far as I know there is no way to read from the pen only and let the mouse continue to work as a mouse - I'll look into this - perhaps I missed something. If I can't work around this, the solution is to attach a USB mouse and set up controllers with that.
If it's a usb/ps2 mouse then there's definately something very wrong with my software - no windows app should freeze the mouse like you describe. Then we have to solve the problem the old fashioned way, me sending you test-versions printing log-files, etc. etc. in this case, drop me an email at jacob [at] livelab [dot] dk, thanks.
Second: you mention the pen can move in the big left hand window - do you mean the right hand window by any change? The big white control surface? If so, then this is perfectly normal - tablet2midi works by taking complete control of the pen movement and translating the pen position to a coordinate on the tablet2midi control surface. The pen is not supposed to be able to leave the control surface.
Hope this helps
Jacob
