When safe mode is broken, I suspect hardware. If I had to guess, my gut would say it's your hard drive is dying or is getting corrupted. It's possible it's a virus, (works once, then is corrupted, you 're-write' the data, and next boot, it's corrupted again?) but it could be hard drive SATA cotnroller, drive, or data cable possibly too.
