transitioning out of a vamp

Any good techniques from transitioning out of a vamp? I frequently get hung up on this. Often time I'll have a vamp that's 2 chords maybe 2 bars each for 16 bars, and then I want to do something else harmonically, but it usually takes me a minute to make it sound like a smooth transition.

This is more of a form related question than anything.

Take the easy way out. Instruments pause, drum fill, boom, new section.
