My K2000S - being my MIDI keyboard as well it definitely gets the most use.
Software-wise, it's hard to pick just one... but I find myself using Oatmeal in pretty much everything, and I definitely do love it. Other contenders would be Scanned Synth Pro, SQ8L, and WusikStation.
