Hi,
I've searched a lot on this subject but could not find an answer. Older VSTi's react on a program change event. Newer ones like Tyrell N6 don't. I've red something about clicking the little + in Reaper. Tried that but that doesn't work for me.
Apart from that, is there some host or plugin that has programmable buttons to quickly change a patch in a VSTi during live-play ?

You have a folder called "MIDI Programs" in Tyrell patches folder. Drop your presets there and they will then react to program changes.

IDK why so many good plugins are unresponsive to MIDI program change, having to reach to my mouse while playing makes me sligthly less interested in using them.
Re. Tal Noisemaker & other plugins:
In most cases you can work around that PITA limitation if your host is REAPER - just click on the little "+" on the left of "Param" in the plugin window and set it to "Link to MIDI program change (Channel X)".
