ok so more than a few things seem off about this.
1. my officialfm page has not been made public to anyone. it is public so someone can go there and see it, but i havent told anyone about it as im not finished with it yet. on the other hand...my soundcloud page is all over the place.
2. the form does not have the officialfm logo.
3. stephanie yamaguchi??? while obviously possible...that name combination doesnt seem likely.
4. .io domain. so...stephanie yamaguchi is sending me an email from the indian ocean? on behalf of a company with offices in new york, geneva, and paris??
5. the form has nothing to do with copyrights.
6. i absolutely own the rights to me music.
ive sent an email to officialfm regarding this, but have not heard back yet.
seems like an odd phishing scheme to me, aimed at an even more odd target base.
what do you guys think?

Very creepy. That "officialfm.wufoo" is not a legit official fm site - it's a form-building site.
I think they're looking to take your music. If you don't have an agent or a publishing house, they might figure you're fair game and the next thing you know your music will be a track in a "license-free" collection being sold all over Bolivia.
Besides taking it up with Official FM, take it up with Wufoo and tell them that their site is being used for phishing.
Tell Stephanie Yamaguchi to come and get you.

I got the same mail... Yes, it's a phishing thingy. I informed official.fm about this and also the hosting site of this form.
Maybe they hacked official.fm and having a copy of the user database with names and e-mails.

Amateurish made school boy scam...someone who isn't even able to create forms without a form builder cloud program... 
I think he (most scammers are young male students) wants to collect the data to sell it to other scammers/spammers, especially the phone number & email address.

All they (I guess: he) made is a (legal!) sub page on wufoo.com (a cloud service for building & hosting forms), not on the offical.fm website. I think this is BS.
Since you know that you havent violated anyones copyrights, perhaps what you should do is ask them exactly who it was that made this claim so you can slap them with a libel action.
Im sure you will never hear from them again...

This is also not allowed by wufoo.com. It seems they having a lot problems with this stuff, they providing a extra item to inform if a page is used for phishing.
...and Afaik the e-mail is not public readable on official.fm and they must combine the official.fm artist name and the e-mail. So there could be a security problem with the official.fm servers.

But I can't get the name Stephanie Yamaguchi out of my head, such a nice name! I hope she's a pretty girl and still available, even if she lives on an island in the Indian Ocean...

This is very strange. I wouldn't call it phishing since that usually involves stealing passwords and account numbers for identity theft purposes. Most of what they are asking for would have no financial value and would be publicly available for many bands.
My first guess is that someone sleazy is trying to build up some kind of spammy mailing list of bands to sell to advertisers. My second guess is that a summer intern at Official.fm was given a task to create a procedure for dealing with copyright infringement and created this wacky form and emailed it to everyone without approval.
Just sit back and wait until you here from Official.fm. It should be interesting.
