I'm looking for something that has phantom power for mics and has the 1/8" out for headphones or speakers.. 1/4' input is fine. 1/4" and xlr is even better.. anything good?

I have this fifty-buck barebones Behringer USB deal that does the trick. You'll need a camera connection kit and a cheapo USB hub as well.

Only problem I have is that Cubasis absolutely hates it -- not only with the app freeze but the iPad refuses to recognize the soundcard until I reboot.

Sonuus has one for low cost, but no xlr. I hate this Ipad, it cramps my hand.

I recently bought a Focusrite iTrack Dock. Haven't had the chance to put it through full trials yet, but it seems pretty straightforward. Phantom power for your midi & mics, two inputs, two quarter inch outputs, headphones. I can't immediately imagine any problems I'll run into, aside from it's input/output limitations. Guess we'll see.

A good review of it here -

If you have an iPhone or an iPad, you have all 3 methods to capture audio—built-in microphone, the headphone+mic port, and a mic that works in the Dock.

Avid Fast Track works fine with original "30pin to lightning adapter" on my Ipad mini:
