Reaper - how can I insert a note into the piano roll with a left click of the mouse?

Making some user errors in setting up the left mouse click to insert a note in the piano roll. I have set the midi editor mouse modifiers to piano roll > left click> insert note and that worked for about 8 notes or so. Then I moved on to setting left drag to move notes. Now left click no longer works to insert a note. SO I closed that midi clip and opened a new one - and voila left click inserts a note. But only one, then it no longer inserts notes.
Obviously I have set up some sort of interaction but I would prefer to be able to just left click and insert a note as many times as I want. I can't think of any time I'd want to have that work only once although obviously there must be cases where people do otherwise the software wouldn't allow it.
Ideally I would be able to insert a note by left clicking and move selected notes with a left drag. If I want to add a note to those already selected I'd use a modifier but otherwise a left click deselects any selected notes and adds in a new one (which is now selected)
Any ideas on how to go about doing that?
The weird thing is how many behaviours I can get happening that I can't see any use for eg left click to insert note which is now the selected note. Now left click again and nothing happens, you can't insert another note. But if you first left click any note the inserted note is deselected and now you can enter a new note with left click. In other words one of the programmed behaviours is "add midi note in piano roll unless another note is selected in which case do nothing" I am assuming that is really useful to someone, but how?

Hi,
In default settings you can insert a note:
1) by left click drag
2) by double left click (note length = "Grid" setting)
3) by pressing "i" (note length = "Grid" setting, note inserted at cursor)
And all work also if another note is selected.

How would I return the midi mouse modifiers to default setting?
also how can I stop the behaviour where the notes remain selected. I really just want to insert a note , then insert another note and the previous notes are no longer selected by the same action as inserting the new note. I don't want to have to insert, deselect, insert
Also there are all sorts of strange behaviours when you want to have left drag to move notes (as outlined above). I am trying to get consistency in the mouse actions where possible - left drag always moving for example, right drag always being marquee select and so on

Left Click - Default action - no action
Left Drag - Default action - insert note drag to extend or change pitch

and... resetting to defaults got what I wanted. I reset everything in all the midi contexts and it worked. Mmm that's strange I thought as the defaults were not working before. So I went and looked at the default setting again and only some of the contexts had reset. Which turned out a blessing. Not sure what are the critical ones as there must be some strong dependencies there that are not at all obvious but
midi piano roll > left click> insert note is now set (which it had been before)
and
midi note > left click > select note and move edit cursor is now set
Interestingly - and I tested this many times - there was previously no difference between midi piano roll > left click> insert note
and midi piano roll > left click> insert note, leaving other notes selected whereas now they do what the descriptions imply.
