Stiv Bators, the singer of Lords of the New Church used to hang himself on stage as part of the final number. He managed to avoid actually exiting that way, though it was close one time, apparently. The time I saw him do it, roadies on either side rushed in to hold him up to release the cable as he kicked the ladder away (it might have been a chair - it was a while ago: the venue now hosts The Lion King).
He wound up exiting in a car accident about five years later.
