Never had such problems myself.
- Maybe the CPU gets overloaded. In that case Reaktor switches itself off.
- Do you have the latest version of SX and Reaktor?
- Do other VSTi's work?
- You're using WinXP?

My first guess is that Cubase is looking at the wrong .dll version.Reaktor will install the VST plugin by default to C:/Program Files/Vstplugins unless firmly told not to(and sometimes even then),and it sometimes creates subfolders in your VST folder.Look in the folder that Cubase is looking in and check the version number of your Reaktor.dlls.
If not the current version, that's your problem.
The quick and easy solution-do a search on your machine and delete all Reaktor.dlls.Then run the 4.13 updater;this time only installing the VST plugin.

I don't have any really good ideas. Which version of Cubase are you running? Whenever I'm having midi-audio problems in Cubase I try increasing the sample latency or (less frequently) playing with the sampling frequency, just to make sure it's not a cpu issue. I used to have odd problems with Cubase SX1 until I installed the Syncrosoft driver update, which is listed separately from the rest of the Cubase patches at the Steinberg site. Shouldn't be a problem if you are on SX2, I think.
What I really wanted to suggest is posting your question on the NI Reaktor forum and/or the Yahoo Reaktor user's group (just found out about it):
http://groups.yahoo.com/group/Reaktor-list/
Apparently a lot of the more experienced Reaktor users migrated to yahoo, you might get some good advice there.
Good luck.
