Live performance soft like mainstage with auto preset switch

I like mainstage, the only caveat is that I cannot change presets by markers... Meaning i have to do a footswitch stepdance every song.
So, something similar, for macOS, that can automatically switch presets - preferably on manually created markers, not on tempo track (since tempo is all over the place and curved)

You may could have a look at Gig Performer.
I don't know if it does precisely what you'r looking for, but its really designed/thought for live set-up.
Just starting to dig into it and already really appreciate what it does.
edit: also really easy onto CPU!

looks neat, but I really don't need anything more than Mainstage3 already does except preset switching linked to the playback

have you looked at zenaudio ALK?
Mainstage has no timeline so I assume by saying markers that you want a timeline...
I've used Mainstage for live guitar looping controlled by foot vswitches, and I've been playing around with ALK for a while as it combines looping controlled by record and play regions pre-drawn on the timeline, on any number of tracks. There's nothing else like it, but it could do with some extra features. Most of which have been flagged as upcoming.

Marker X switch to patch Y
so a playback track, and # of sounds for synths and live audio inputs. No looping, nothing fancy. When it hits Chorus it switches to patch chorus which changes the sounds needed for the chorus.
in mainstage I do this by foot, but it's cumbersome and i need to memorize the parts for the whole concert and it's a pain in the ass

Oh I didn't know about markers in that sense. You could actually use ALK for this. You'd load your audio file, make separate tracks for all your different sounds, and draw record loops where you wish to play each one live. Even though you won't actually loop them, this is how you'd get each track to sound at the appropriate time.
Then playing live you'd hit record, your playback loop would play, and as you play live over the top, each sound you want would activate/deactivate as the song progresses. A record loop can be as long as the song, and doesn't have to ever loop.
I'm pretty sure it would do what you want quite easily despite it not being aimed specifically at that. Free trial and the 'solo' version is pretty cheap, so maybe worth a look.
Though, one of the things it's missing now is a 'concert' feature, so it would mean loading a new song each time. But that feature is coming

I am pretty sure you can do this using instrument racks and chaining. A friend demonstrated to me as he uses this technique for his performances. I don't totally recall how he did it but it requires a MIDI recording I believe. When you want a new sound, you tell the track to move to the next chain. He also pre-recorded things like volume fades and modulations so he only needs to focus on playing and the Ableton track does the rest. He was using Ableton as the master for click and tracks as well so it was all perfectly timed together.

Have you had a look at scripter? It is capable of retreaving timing information, which in turn could send a program change. Its javascript though...
And as it sounds you don‘t want to learn a new host, this might get you there.
Or you play your playback as pure midi, which could have program changes embedded...

I checked the scripter! this seems the most elegant way to do it, and learning a whole new host (and paying for it) seems to be an overkill.
