An Akai S1100. Forgot it was an US version and I plugged it in to a EU socket.
Thankfully it was just a blown fuse.

I think I blew up my ART SplitMix4. I don’t know how - it’s a passive mixer. There isn’t anything to blow up. But I get nothing but hum from the output when anything is plugged into any input. There must be a short inside somewhere.

LSC Precept 36 prototype lighting console. Long time ago.
I'd just finished programming a world premiere show into it when I dropped my coffee on top of the thing.
80 odd thousand watt flash from the stage then complete darkness and a rather quiet "Oh" from yours truly...
I had three hours to find another lighting desk before curtain.
The Producer suddenly looked very ill ...
I found a board and ran the lighting manually making it up on the fly...

2.5K repair bill.
I'm tired of being insane. I'm going outsane for some fresh air.

When I was a kid I blew up my Hot Wheels charging station. The batteries had gone flat so I tried plugging it directly into the mains. I was electrocuted and thrown across the room and the Hot Wheels charger went up in flames ... I've never blown up any audio gear but I'm a bit more careful now :hihi:
