What is out there?

Looks like I'd have to transpose at a software level as my Novation Impulse octave buttons aren't tied to CC numbers.

They never are. Midi is supposed to work with absolute note numbers.

I do see some possibilities:
  • DIY. Rig up two sustain pedals, hard-wire them to the transpose buttons
  • Planning! Avoid having to press transpose while you need two hands playing
  • Get a bigger master keyboard (88 keys instead of 48/61)

That sounds like a great idea but I don't think you'll find anything. You might be better off automating the octave tuning in your instrument.

MIDI Solutions Footswitch Controller should work. It has a foot-controllable transpose function. Use the Dual model if you want two functions (i.e. octave up, octave down). https://midisolutions.com/proddfs.htm

Doesn't directly double as octave transpose buttons (-2 / -1, +1, +2) but nice find nevertheless!
