I have the VTB-1 and it does a good job on vocals. I'm definitely happy with it for the price.

However, I'm not sure if it is the best for instruments. A inexpensive solution for instruments is the M-Audio Audiobuddy. Guitar Center had those on sale for $49 a couple of months ago. They may still have them at that price.

The RME the one poster is selling will likely blow the VTB-1 out of the water, but it is also twice as much. If you have the money, it's probably the way to go. If it is out of your budget I think the VTB-1 and perhaps an audiobuddy will cover everything you want.
Formally the mephit called DrakeBP

I could recommend the Manley and Chandler I was playing with a few days ago. What's your budget though? ;)

I have a Behringer MIC100 that I use for vocals and guitar. I love it, but I've got nothing to compare it with, so don't take my word for it:

I use an M-Audio Audio Buddy. It's a 2-channel pre-amp and each channel can be either Mic (XLR connector - switchable phantom power) or guitar/bass.

I find it very useful, am happy with the sound quality and it has never let me down :)

I wish I could make or buy a battery pack and have the AudioBuddy be portable. Ideas?

well, the input is 9VAC, so that's going to be difficult.

I would imagine it converts that to some DC rails inside - if you could get a circuit diagram, you might be able to figure something out. :?

Samson C-Valve. Highly recommended for Vocal or instruments/Guitar.
