I think I know why you're uncomfortable with that, but if it makes you feel any better, "girl" does not always mean someone who is below the legal age of consent. It's fairly common to refer to a 19-year old as a "girl". I've heard the term applied fairly often to women in their early 20s, especially if it's someone older making the statement. I don't know why Bob phrased it that way. Let's hope, it was purely to make the line have enough syllables to fit with his rhyming scheme.
Also... I suppose, "girl" could have been a last-minute replacement for "man", because his record label might have been uncomfortable with that?

Funnily i think the line "Visions of swastikas in my head and plans for everyone" works in Iggy Pops version with his snarly delivery but is just plain wrong in Bowies rendition of the same song.
