connecting my m-audio fast track pro up properly

im haveing a gard time trying to figure out how to connect everything up
this is what i want to achieve
connect my small mixer up to the soundcard also monitor in headphones
here is the kit i have
a small folio notepad mixer,m-audio fast track pro, a sony amp
would really appricate some help. I really am not very good when it comes to connecting things up
