Not double-checking that the Monitor button on every track was off before exporting audio tracks from Nuendo 4.
Which, BTW, is time-consuming as all hell, since you have to export every goddamn track individually.

The most stupid thing I did was probably using vacuum cleaner in room with guitars on floorstand - a carpet moved and my Gibson LP Standard fell over onto floor and headstock broke.
But it was a clean brake and fully recovered when mended. But I was devastated at the time.

Hours diagnosing why my Nord Lead stopped responding to knob movements only to discover I had replicated the part on a VST.
