synth example 8.0.4 in sonic birth manual

Howdy,
I am a newbie, and am trying to set up the example 8.0.4 in the manual, which is supposed to take midi input, pass it through a couple simple elements, and output. As near as I can tell I have everything set up right, but I hear no audio when I press a key.
Versions: Sonic Birth 1.3.0 (free download, not registered). Mac OSX 10.4.11. Intel Mac a couple years old.
My setup looks identical to that in figures 17 and 18, with two exceptions. First, I added a constant zero as input to the swave p input (I did this only as part of a desperate hunt for what might be wrong-- I think it makes no difference). Second, in the Midi Setting box in figure 18, my third row says "off" instead of "N/A". I get a popup menu there but no matter what I choose it still says "off".
The "midi server" window has a popup menu for source, and it allows me to select "Evolution Electronic Ltd. :: USB keystation 61es :: USB keystation 61es". This menu item comes and goes if I turn the keyboard off and on. So I conclude that the connection from my keyboard to sonic birth is ok. I also am able to hear notes if I run garage band, so I know the keyboard is working. And I was able to hear output from the manual's first three examples (which do not involve midi), so I know sonic birth can drive my speakers.
Anyone have any ideas? IS there something I missed in the manual, like midi won't work unless I pay and/or register?

I am using OSX 10.4.11. According to the download page (sonicbirth.sourceforge.net/download.shtml) the requirement is 10.3.9. I assume that means 10.3.9 or newer.
It also says QuickTime 7 is required. How can I figure out what version of QuickTime I have? My QuickTime Player says it is 7.5.5, but I don't know if that number relates to QuickTime itself.

hiya mate!
i had the same problem...what you have to remember is that you have to start the sound server when your patch is in runtime in order to hear anything.
so:
- make sure all you audio/midi settings are correct
- make sure your patch works ! (mebbe test with a demo patch?)
- set the patch to 'runtime' under its settings
- open the 'sound' window and click 'start'
- push some keys (or whatever is necessary to trigger sound)
and now you should be able to hear sound. winner!
