I've seen many others beside myself that has had the same problems with this bank., but have not seen an answer or fix to it.
*Patches producing only intial sine waves.
*Patches that can sporatically go wild and clip speakers forcing to restart the engine to clear it.
I'm getting ready to take this off completely, but would like to give it every chance before I do.
Was the Insigna sound bank ever fixed?

I thought it was mainly the new soundbank from Kevser that had the sine waves where the drum patches should be. This happened with the 2.6.9 install for me. I also had two DR patches from the previously installed Insigna Trance sound bank that were fine until I went to 2.6.9 but they somehow corrected themselves. To my knowledge this has not been fixed but I would sure like it to be.

Do you see the following folder:
<synthmaster root folder>\Expansions\Insigna Essentials 4 Clubbin\Samples
I've checked both installers for Win/Mac and the folders/files are installed correctly. Have you tried downloading the latest installer?
