These red entries say that they are "deactivated after failing to initialize correctly"
I tried highlighting and remove selected from list, I cleard list and rescan. Stopped and started T4, and its still there even though the plugin was removed when I first got the red words.

Just highlight the ones you want to remove and press the backspace key.
They don't get removed when you clear the list because they'll likely crash subsequent scans.
