Hey that's the UAD-1 card! But it says WinFast3D
It's really a Cromatic Research’s Mpact 2, a 125MHz, graphics, audio and video media processor, used primarily for AGP graphics cards and hardware DVD decoding in 1997. Yes this is a fact. In small print on the UAD-1 it says "Copyright (c) 1999 / Hyperactive Audio Systems Inc". I guess this card was discontinued by Chromatic Research and UA picked up boatloads of them for cheap. Using sisoft Sandra reveals that the UAD-1 and this WinFast card have the same VendorID, ProductID, and RevisionID.
