Triple Cheese VST going haywire

When I use the Triple Cheese VST, it randomly glitches and goes to max volume (and nearly deafening me). It happens on different channels with different presets. With other effects on or off. I don't know what to do. A month or two ago it would happen once a week. Now its happening every time I play the channel. Please help.

Perhaps it would be useful if you told us what operating system, DAW and version of Triple Cheese you were using.

Whenever you load an instance of TripleCheese the version number should appear in the data display. Mine says "TripleCheese 1.2.1" there, and "Rev 4446" at the bottom.

Please don't use the VST3 version of Triple Cheese, that's what the VST3 readme file in the installer is about.
Use VST2 or AU with Ableton Live.

It says Rev 3898 on the bottom but i don't see anywhere where it says the version number. At the top of the window it says "TripleCheese/9-TripleCheese". the "9-..."part is just the name of the channel.

As Howard already said, when you load a new instance, the version number is displayed in the main data display in the middle of the GUI. But the revision number is already sufficient information.
Again, please simply don't use the VST3 version, as this one is causing your problems.
