Funny thing is, no one really gave a rat's hoo-haa about it when it actually came out.
The not-subtle message on capitalisation was noted, but no one really regarded the cover as taboo, until quite recently.
I remember seeing about 200-300 copies sitting on a stand at Brashes, at the time of release, with an absolutely gigantic poster of the cover over it... Simply put, it just wasn't regarded or considered wrong, for the longest time.
The band signed off on it, so did their label, and lots of people bought it...
To question it's artistic validity, is to directly question Cobain's artistic integrity, because I'm sure, he created the artwork from the photo (I might be wrong on that)... Yet no one has done that.

You could put a picture of a camel smoking a blunt while reading cosmopolitan on the front of that album, its still one of the best of all time. (Although I didn't truly appreciate it at the time)
