I noticed that adding an effects plugin to the mixer channel [add plugin to the left] will create the instance of the plugin but the plugin buttons on the mixer channel disappear. This behavior was recreated multiple times. Closing and reopening the mixer view seems to resolve the issue

Where is the [add plugin to the left] in the mixer*? Which plugin(s)? What is a plugin button? Which version of Waveform? Which OS?
I tried a few things that might be a match for what you're describing (added plugins on the left on the track, added plugins at the bottom on a mixer track) but everything seemed to work as expected. Waveform 12.0.53 running on WIndows 10.
* EDIT: Found it. I've never used that method before. I always drag and drop from the Browser.

@union28, I also confirm the behaviour that you have shown in your video. I am on a iMac running Catalina. Closing the mixer and then reopening it brings things back to normal.

In your video, I don't see the plugin menu open. I'm trying to reproduce? Click on the plus, drag to a track and all the plugins disappear?

On my computer reordering already present plugins eighter on outputs panel or in mixer strips causes all plugins in the mixer disappear. Not adding, just reordering. Perhaps I misunderstood the OP. I'm sorry. But nonetheless there is also a bug

The videos are very helpful, thank you. I was able to re-create it once or twice, but it doesn't seem to be consistent for me. Ctrl-click drag and drop a plugin into a space between plugins in the mixer made the plugin icons in that mixer channel all disappear (visually).
Surely there must be consensus by now...

This is another video confirming this behaviour. I click on the plus in the mixer area to add a reverb to the vox track. The plugin gets added below the Volume and Pan plugins. I then drag the Reverb plugin to move it above the other 2 plugins and then all the plugins disappear. It is only when I close the mixer and re-open it where the plugins are viewable again in the mixer. Let me know if the link for the video provided opens okay.
