I have a question regarding that. Does one have to upgrade whenever switching to a newer version? Like from 4.2.2 to 4.3.1?

You don't "have to". It's up to you to decide whether the changes (new features, improvements, fixes) interest you.

Not sure if we are talking about the same thing here. I meant, do I have to or does it make sense to upgrade my 4.2.2. sessions when I upgrade my Mulab, say from 4.2.2 to 4.2.4. or 4.3.1?

So far all M4x.x sessions are compatible.
The last step was from M3.2.14 to M4. Meaning that M4 can load session made with M3.2.14 and later. So, it's a good time to convert any M3 sessions now.
The same for instrument/effect patches. M4 even comes with a conversion, so that M3 patches can be loaded.
Jo will usually tell us when the file structure will change.
