I am running Omnisphere-3 in Ableton Live 12/Win10-Pro (latest).
I can load Slot1 and Slot2, then Slot3- up to Slot8 makes no sound. No matter what channel and output selection I set. Also, when I play a key I only see SLot1 and 2 flashing slightly (indicating MIDI coming in).
I have another instance of Omnisphere on another track with 4 Slots active with 4 patches and all of them are playing as expected.
Any ideas what the problem is ?

While doing some extra testing I discovered that Multi works OK on a new track and when I create new Multi-Preset everything works OK. The problem mentioned applies to all my existing Multi's (and I have quite a few). Even Multi Preset created with Omnisphere-3 just a few weeks ago does not work. All of them will only play first 2 slots, no matter what I try the remaining 6 slots make no sound.

I am not at the right computer to provide screenshots, but hopefully you can understand from my description.
1. Switch from the Patches menu to Multis menu.
(Drop down arrow to the left of "Patches" text)
2. Click on Refresh button underneath
(Looks like a "replay" button, beside the "Play button"
That should fix it.
BTW if your patches need to be refreshed you do it the same way but in patch mode not multi mode.

My main tools: Kontakt, Omnisphere, Samplemodeling + Audio Modeling. Unify = godsend. Tari's libraries also rock.
