It's not very easy to find out without seeing the code, but be aware that the position given by the transport interface is the position of the beginning of the audio buffer. You then have to make sure the timestamp for the note event is set properly.
It's not the cleanest code ever, but have you checked the MIDI metronome sample script? It sends MIDI note events on each beat.
